Greater Health Now Mobile Integrated Healthcare: Delivering care outside traditional settings

Since 2022, Greater Health Now has funded Mobile Integrated Healthcare programs that bring care directly to people across the region. This program uses paramedics and community-based workers to deliver care outside traditional settings, preventing unnecessary emergency department transports. This program connects people with the emergent health care they need, while linking them to services and resources to meet their basic needs, like food, emergency housing vouchers, home safety checks and more. This program drives cost savings and leads to better health outcomes.
